Able Home Healthcare is looking for a passionate and skilled Home Health Occupational Therapist to join our growing team in San Diego, CA. Responsibilities:
- Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans for home health patients
- Provide skilled OT interventions focused on ADLs, fine motor skills, cognitive rehabilitation, and functional independence
- Monitor and document patient progress, adjusting care plans as clinically appropriate
- Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated patient care
- Educate patients and caregivers on therapeutic techniques, adaptive equipment, and home safety modifications
- Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
- Manage a caseload of home visits throughout the San Diego area
Requirements:
- Master's or Doctoral degree in Occupational Therapy from an accredited program
- Current California OT license in good standing
- NBCOT certification required
- Home health experience preferred; new grads with clinical fieldwork in home health encouraged to apply
- Valid driver's license, reliable transportation, and current auto insurance
- Strong knowledge of Medicare home health guidelines and documentation requirements
- Excellent communication, time management, and organizational skills
- Compassionate, self-motivated, and comfortable working independently in the field

Compensation: $85.00 - $120.00 per hour
