Able Home Healthcare is looking for a passionate and dedicated Home Health Physical Therapist to join our growing team in San Diego, CA. Responsibilities:
- Conduct comprehensive in-home physical therapy evaluations and develop individualized treatment plans
- Provide skilled PT interventions including therapeutic exercise, gait training, and neuromuscular re-education
- Monitor and document patient progress, adjusting care plans as clinically appropriate
- Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated patient care
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
- Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
- Manage a caseload of home health patients across the San Diego area with a flexible schedule
Requirements:
- Active California Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program (DPT or MPT)
- Home health or outpatient orthopedic/neurological experience preferred
- Strong knowledge of Medicare home health guidelines and documentation requirements
- Excellent communication, time management, and organizational skills
- Valid California driver's license and reliable personal vehicle with current auto insurance
- Compassionate, self-motivated, and comfortable working independently in a field-based setting

Compensation: $85.00 - $120.00 per hour
